Job Title: NP Geriatric Medicine

Job Summary:

We are seeking a skilled and compassionate Nurse Practitioner (NP) specializing in Geriatric Medicine to join our esteemed healthcare facility. As an NP in Geriatric Medicine, you will work closely with a team of healthcare professionals to provide comprehensive care to older adults, addressing their unique medical needs and promoting their overall well-being. The ideal candidate will possess a strong clinical background and excellent communication skills to effectively interact with patients, families, and other healthcare providers.

Responsibilities and Duties:

1. Perform comprehensive physical assessments and diagnose medical conditions in geriatric patients.

2. Develop and implement appropriate treatment plans, including prescribing medications and therapies.

3. Monitor and manage chronic conditions, such as hypertension, diabetes, and heart disease.

4. Conduct and interpret diagnostic and laboratory tests specific to geriatric patients.

5. Collaborate with interdisciplinary teams to coordinate care, ensuring a holistic approach for elderly patients.

6. Educate patients and their families about disease prevention, management, and health promotion.

7. Provide counseling and support to patients and families facing end-of-life decisions and care.

8. Assess the functional and cognitive abilities of elderly patients for appropriate care planning.

9. Assist in the coordination of transitional care and referrals to other healthcare providers.

10. Document patient encounters, treatment plans, and progress notes accurately and promptly.

11. Stay up-to-date with latest research, guidelines, and advancements in Geriatric Medicine.

12. Participate in quality improvement initiatives within the department and organization.

13. Maintain a compassionate and empathetic approach towards geriatric patients, recognizing their unique healthcare challenges.

14. Adhere to legal and ethical standards in nursing practice, focusing on patient privacy and confidentiality.

15. Foster a collaborative and positive work environment for effective teamwork and professional growth.

Qualifications and Skills:

1. Master's degree in Nursing with specialization in Geriatric Medicine.

2. Current RN and NP licensure, along with Board Certification as a Nurse Practitioner in Geriatric Medicine.

3. Proven experience working as an NP in Geriatric Medicine or a relevant field.

4. Solid understanding of geriatric syndromes, cognitive disorders, and management of age-related diseases.

5. Excellent communication and interpersonal skills to effectively build rapport with elderly patients and their families.

6. Ability to work independently and as part of a multidisciplinary team, demonstrating strong leadership and collaboration.

7. Proficiency in conducting physical assessments, interpreting diagnostic results, and formulating appropriate treatment plans.

8. Strong organizational and time management skills to prioritize patient care and meet deadlines.

9. Proficient in medical record documentation and familiarity with electronic health record systems.

10. Commitment to ongoing professional development and continuing education to stay current in the field.

11. Understanding of Medicare regulations, healthcare policies, and reimbursement mechanisms in geriatric care.
